You’ll submit two completed assessments called the Generalized Anxiety Disorder 7 (GAD-7) and the Patient Health Questionnaire-9 (PHQ-9) at the end of each session, right from your client portal.

It is a 7 item questionnaire used to assess symptom categories associated with stress or anxiety. This assessment helps you better understand the frequency, intensity, and impact of any symptoms you may be experiencing.
Over time, you’ll be able to understand your overall level of anxiety symptoms, any changes to those symptoms, and the impact of those symptoms on your functioning.
It also tracks your day-to-day functioning over time. You’ll answer three questions about day-to-day functioning and one question about symptoms. The final scores show your perspective of how you’re functioning has changed over the last 2 weeks.
It is a 9 item questionnaire used to assess symptoms associated with depression. This tool will help you better understand the frequency, intensity, and impact of any symptoms you may be experiencing.

You’ll answer nine questions about a range of depressive symptoms. The final scores show your perspective of how you’re functioning has changed over the last 2 weeks.
